If you look carefully, there's also a bunch of naked ladies in the smoke stuff (the same stuff that the skull is made of)...

Myself, I think the cover shows a llama emerging from the Eye of Sauron.

To me the thing he's standing on looks like a woman looking to the right, you can kind of make out eyelashes and a nose :D
