    If you think it's coming from your back, I would still focus on treatment on the back. If you want to try it on your foot, depending on how big the pads are, maybe the top or bottom. Sometimes it takes a few placements to find the best spot that works for you.

    EMS helps retrain the muscles, not necessarily gain muscle, but it will help to get them working correctly again. Are you going to physical therapy now? They should be able to show you the best way to do it.

    AskDoctorJo no because my physical therapy specialist advised me to do gym to gain more muscle which was a bad recommendation and unprofessional which led to injur my MM again and now im stuck I cannot do another surgery because i have weak and small muscles and i cant build them thru exercises because i have the MM tear and i have pain every time i exercise.
    I tried to do some easy types of exercises for the last year and half and i gained 0 muscle massa d strength i have took supplements and also didn’t get any results .

    I'm curious your opinion on NeuroMD & the Compex Sport Elite. I think they work differently than TENS.

    I haven't tried either of those, but looking at them, they are different than TENS. NeuroMD says its NMES, which stands for neuromuscular electrical stimulation. The Complex Sport Elite states it's a muscle stimulator, which is often termed as EMS. These would be what you would use for retraining your muscles.

    Most of the ones you can purchase online are going to be pretty much the same, no matter the price. There are two classes of units, one is the kind you purchase online without a doctor's prescription, and the other is one you would get in a heathcare clinic with a doctor's prescription. They are more expensive, but usually stronger, and sometimes insurance will cover them. But when used correctly, I have found that a $30 one works almost as well as a $1000 one. You are paying for bells and whistles at that point.

    You're welcome. Most of them have generic pads (they recommend you buy their brand, and you should if you can), so you could probably get them from anywhere for a long time.

    I would reach out to the company for that question. They might have cords with more channels that you can just plug into the same outlet, but unless the unit says that it has more than 2 channels, it probably doesn't do that. Also, if you are wanting to use EMS, you should only use two electrodes (1 channel) at a time anyway.

    I am not familiar with that particular machine, so I don't know specific numbers for it. For the EMS portion, does it have a specific name, like Russian current, or are they all pre-programmed?

    My PT is using both of these types of therapy on my knee ...sadly the quad muscle is not contracting...I allow the PT to increase the intensity nearly beyond what I can tolerate so we can get this quad to contract but still no contraction

    It might just be the electrodes need to be moved around some. Sometimes it takes a few tries in different spots to get on the right area.

    If you are working the TMJ area, I would not recommend EMS. The TENS units usually have several different modes, and most likely one would be able to help with that.

    I always love your videos-very informative! I do have a question: is EMS good for muscle knots/spasms? Would the electrical contraction from EMS reset the already over firing of the muscle fibers or make it worse by contracting more? I use a TENS-it helps, but I'd really like to relieve knots so I can exercise to build up weak muscles in my lower back due to scoliosis.

    Hello, my doctor is suggesting i use the kneehab for my ACL/ Meniscus Surgery. It is a bit pricey, what do you suggest to use instead that is more affordable.

    As long as you are going to physical therapy for treatment, a TENS/EMS unit should be almost as beneficial. You can ask your therapist to help you set up the EMS part on your unit to get the best results.

    I use the smaller device. In fact, I had a high volume squat session three days ago. The DOMS was bad because of the new stimuli to the muscle group. Last night I put the large pads on the leg in the direction of the actual muscle pattern. I had it on the second highest setting to really dig in. It felt amazing and after I felt so much better. Today, the quad pain is almost non-existent, I also added it to the calf area, not as effective but a bit better today.

    So it turns out I did tear my quad tendon while sprinting. I feel as if my tendon is still dangling and when I lift up my quad I get a dent in the middle of the quad. The sports orthopedic told me he feels I don't need surgery the tendon can heal on its own so he's sending me to these therapies with the electrode machine. I feel it's healing but incorrectly, should I baile out and change doctor's. What's your opinion?

    Unfortunately, I really don't know since I can't evaluate you. You will need to do proper stretches and exercises for it to heal too. Just electrical stimulation won't be enough.

    It's really to your comfort level. So I usually recommend one that slowly ramps up and down, not like the acupuncture setting, which sometimes causes a quick contraction.

    No, they all do pretty much the same. The price is often based on "bells and whistles." Like is it a touch screen does it have 8 modes or 16 modes, etc. But the $25 one usually does almost the same thing as the $100 one.

    @AskDoctorJo no direct injury..just ergonomics throughout life..I'm a little taller than average and after an hour or two of sitting or doing anything my shoulder and arm start pulsing in like nerve pain..it is so aggravating..I bought a stand up desk but still have the nerve pain in other situations..I just bought a tens unit..hopefully this could help..but not sure what else to do or who else to see..a chiropractor, physical therapist..not sure.

    I would not recommend using pads that are not specifically for a TENS unit since they might not be compatible.

    Dr. Jo, could you please, please make a video on units that have massaging functions to relax the muscles? Not to exercise, not for pain, just muscle relaxation.
    The info available online, as well as the standard settings on the units are often contradicting as to how many microseconds should be used depending on which part of the body it’s applied to as well as which frequencies are best (5Mhz< and such).
    Your opinion would be greatly appreciated.

    There are a lot of variations to it (usually anywhere from 50-250 microseconds, with 50 - 100 Hz for the "normal" setting), but I would stick to the manual since each unit is different. It really is to your specific tolerance, so that's why it varies so much. What might be comfortable for you, might not be for someone else. So I would really try different settings and see which one relaxes you the most. As long as you don't have the intensity too high, it shouldn't hurt anything.

    Can I ask you something else? If a muscle is too tight, can it be relaxed without reaching the point where there's a contraction?
    I read somewhere that frequencies lower than 10Mhz send oxygen and restore the fibers by removing "waste", which then relaxes the muscles.
    I'm afraid of seeing a contraction or using the EMS function because I don't know if that could cause a tear or something.

    Hz or Mhz? I have only seen ones with Hz, which is also pps (pulse per second). Usually a lower Hz is for a different setting, which sometimes does cause a quick contraction. But again, all units are pretty different, so I am not sure with yours. With that said, a unit can not produce a contraction stronger than what you normally can (without injury), so unless you just leave it on with a painful contraction for a long period, it is not likely to tear anything. It might make it sore, but it shouldn't be anything bad.

    reading it, I realized it has an internal battery,
    probably Ni-Co; which is able to recharge 50,000 x {or cycles
    as some would say.} That means to me, that there
    are screws on the back cover, that would allow
    a knowledgable person to remove, carefully remove
    the battery. Now that single battery is actually multiple
    batteries or a paste(aka cell phone/ gps ect). If a paste, you buy one similar and
    solder it back in. If it is multiple cells; (aka A, AA, AAA types,)
    you order the exact number cells, solder them together and then
    replace the package exactly as you first saw it. Yeah, this is the one
    I will save up and buy. Thanks, wishing you the best Xmas ever and

    Most chiropractic offices seem to use (EMS) when people are injured (car accidents). When you’re injured, you feel pain right…? So seeing that EMS is for muscle rehab, why does EMS seem to be used more often in a chiropractic setting than TENS?

    The way I see it, is that people feel pain regardless; and anyone looking into an EMS / TENS unit, is probably doing so because they feel pain somewhere. So it would seem as if TENS should be used in any case. How do you know if you should use TENS (block the pain) or use (EMS) stimulate the muscle. TENS sounds like a temp solution, but what will EMS do for the long run?
    And to clarify that question, I guess I’m basically just asking, how do you know if you need a pain management solution vs a muscle rehab solution…?

    I can only assume that you have to determine whether or not your pain is a muscle related issue or a nerve related issue…

    I don't really know what most chiropractors use, but many of my patients talk about some form of TENS being used if they see a chiropractor also. They might call it "e-stim" which can be a number of different currents, but if you are not getting a muscle contraction (or at least trying to), it's not EMS or NMES. EMS can be used for pain control as well since technically retraining a muscle should help decrease the pain. So it is really injury dependent. The best way to find out what you need is to go see a health professional first, even if it's just for an evaluation.

    I have MS and need to avoid atrophy of right leg muscles. I swim every day but I don't think the muscles are getting enough stimulation. Could I use a TENS machine to help?
    I tried the Walk Aide that shocked the peroneal nerve when the knee was bent but found it pretty distracting and painful as it jerked my drop foot with the shock. But I liked the idea of stimulating the nerve- just not the price tag of $5K. The TENS units are much cheaper and would they do the trick?

    TENS units are really for pain. EMS units will help contract the muscles, and might be worth trying.

    They are all pretty different, but with TENS, it's really about what is most comfortable for you since it is for pain control.

    Hello! I have a question please. I am suffering from full body atrophy. Doctors are clueless but I know it's from a spinal injury I've had years ago. Anyways, I am wasting away so I have been trying to combat it with weightlifting but I can only workout certain muscles at the moment due to lack of workout equipment. Will the ems unit help my muscles to stimulate and grow? Thanks.

    It really depends on how much of a spinal cord injury you had. It can help retrain muscles, but it will not grow muscle if they are not able to grow on their own. Since they are fairly inexpensive, it might be worth trying though. Good luck.

    What do you mean by "one?" The muscle that contracts or the unit that does the contraction? Unfortunately, each unit is usually pretty different with it's settings.

    last week i had a rt shld slap tenodesis / re-anchor, and further cleaning up inside joint (2.5 hrs surgery). For day 2 - post-op was PT, and my therapist was very pleased with my state, nonetheless it was at this session where the t.e.n.s. unit was used for 6 min. Was stimulating in most areas, BUT ever 5 seconds or so I experienced this randomly sharp painful twitch sensation inside the joint, and it was stemming from i believe the electrode placed on the teres minor area. Can't describe it any better other than feeling red-flaggish. Anyhow, I am on day 4 of recovery now, I will mention this with my surgeon in 8 days. I am curious though, Is this a normal anomaly with the unit? Could this be related to something else, like an additional tear or nerve issue outside view of contrast images?

    You're welcome! I don't think it is any of those possibilities. TENS really stays on the surface nerves, and doesn't usually activate the muscles around the joint. It's possible the TENS was on a little too high causing occasional contractions, or maybe the electrodes just needed to be moved/adjusted (sometimes if they are right over certain areas of the muscle, it can be painful). If you have another PT session soon, definitely let your PT know, and they can problem solve with you, but I wouldn't be too worried about it. Good luck!

    Hello. I have a herniated disc in my back (L5-S1) and am experiencing a lot of pain. Sometimes I can barely walk or move it is so uncomfortable. Consequently, I'm getting way out of shape and its increasing the back pain even more. Can I use one of these while I work out? I'd like to walk or try something for cardio. I'm dying. Help!

    Surgery was March 12th 2018. I am out of my cast and into the CAM boot which I can take on and off but can not weight bear unless I am wearing it. I have started with PT and I am doing exercises without the CAM boot while at PT to regain mobility and strength. I am currently at a 1 inch calf difference and PT said that will only get worse. I am a very fit person who would really like for it not to get worse so I'm hoping EMS with as tolerable seated calf raises might help. Thank you for your response.

    I have a pinched C6-7 nerve, resulted in pretty noticeable lateral tricep head atrophy. I went to PT. The neuropathy is gone, but the muscle will not activate during exercises that should make it contract- it’s all medial head working. I saw orthopedist for possible surgery. He thinks I might recover, but I’m still very weak in the tricep. No one has mentioned an EMS to reverse this wasting. Are they just not educated or are these devices modern day “snake oil”? Thanks

    They definitely work, but it really depends on the injury. There has to be a nerve signal getting to the muscle for it to work though. I think it's definitely worth trying. Usually in the clinic, we use a Russian current to try to get activation. I would ask your PT. You might not be a candidate for it, but it's probably worth giving it a go at least once.

    Yes, since TENS is for pain control, you can use it several times a day 3-5. EMS is actually working the muscles so you don't want to over work them, so probably just 1-2 times a day. Good luck.

My understanding is that you put one electrode down on the Pt. and then one in the palm of your hand. You then turn the machine up to a light intensity and run your finger down the muscle until you feel the electricity at its strongest point? Is that the (safe) way to go about it? Thanks.

    If you don't have a pen-electrode, you can do it that way. You might have to wet your finger slightly for the current to go through, but it won't hurt you either way. Good luck.

    I have been bedridden for a few months with an impinged nerve in lower back which means can't stand or sit for no time at all hardly. I ordered a tens machine off amazon to help with my lower back muscle that has been basically locked on for these past months due to the impingement. I forgot about my legs wasting away until today they are aching real bad, I never had developed leg muscles to begin with (chicken legs) so really didn't have much leeway I guess for being laid up this long.
    Would the EMS unit help me to rebuild these atrophied quads? Or would I be ok with using those exercise bands to work the muscle and then use tens to make the ache afterwards disapate?

    If you are able to contract your muscles on your own at all, that's what you should try to do. You can do something like a quad set or a straight leg raise with the EMS, but you always want to try to actively move your muscles as well. Good luck.

I know they can be pricey but im sure you can find a good affordable & i promise you won't regret it if you find 1 who knows what thwy like are doing. for the pain though i dont see why the unit wouldn't help.

I am 44 years old and I had my Hip replacement 6 years ago, it helped a lot with mobility and it extended my right femur improving the length of my leg, it was shorter since I broke my femur when I was 9 years old in a car accident. Usually I can walk without pain but as soon as I walk more or try to exercise, I start feeling pain. I just bough this machine to improve my legs muscles for strength and better appearance.     They all do very similar things, but in very different ways. TENS and EMS are actual currents that not only help increase circulation to an area, but it also help relieve pain. The Thermotex infrared heat uses light therapy to help "heat up" the area which increases circulation and helps relieve pain. So it really depends on your specific needs and how far out you are from your injury.

    Depending on the electrodes, you can cut them to be smaller as long as you don't cut the part that has the wiring. Then you can get it closer to the area since it's smaller. Do you have the square shaped electrodes?

    Yes, I have the 2" square electrodes, I place one on the palm of my hand and one on the joint above the thumbs base, Set it to 120pps and width at 50. I have very large hands, most gloves I have to buy are X-large. I also use the compression gloves at night.

    It might help some, but usually EMS is for when you really can't activate the muscle much yourself, like after a surgery or serious injury. Good luck.

    Most of the combos are pretty good, but you can tell a difference with the contraction if it's just an EMS unit. It really depends a lot on your body type as well. People with less adipose tissue (fat) tend to get a stronger contraction because it goes straight to the muscle. Good luck.

    Hi there Dr Jo,
    Back in September 2nd 2015 I had a heart attack and ended up having an emergency operation.
    I had a triple heart bypass and the vein used was from my left leg, cut from ankle to just above the knee.
    Since then I have had knee aches and pains and back aches and also a lot of gout too.
    One good thing about it is that I am still alive though. Would the TENS benefit my aches and pains? It is painful when I walk now on one side of my hips but I do my best to plod on.
    I am 67 now but still young at heart and I want to be able to get around without knee and hip and back pains.

    I have been experiencing numbness in my toes and have attributed it to my walking regiment of 7 mile walks 5 days a week. I have found that re lacing my shoes has removed the pressure from the nerve that runs from the top of my foot to the big toe but the neuropathy lingers.     I used this a while back because of shoulder pain unfortunately I left the machine on my body for more than 15 Mins and I started to feel my heart pounding, out of breath and faint that when I tried to turn it off the settings went up. After that I started to notice that I would get spasms, heart palpitations and chest discomfort, I've been told that all my symptoms are due to anxiety and panic attacks and there is no correlation with the TENS machine, could I have damaged my vagus nerve or something else? I can't wear tight pants, I can't stand for too long and no matter what I eat my stomach boats and my heart goes out of control.

    I don't think there is a relation. I have used a TENS for my shoulder when recovering from a surgery, and I kept it on for well over 30 minutes. But I really don't know without getting to evaluate you.

    My friend bought me a a tens unit knowing that I wanted an EMS but I don't think that she knew the difference. I understand the difference between a tens unit and an EMS but have a question about my tens unit. Although mine is exclusively a tens unit, it has the capability of producing forceful muscles contraction when I place the electrodes on my VMO and foot arch, for example. This works when I put it on the "Burst mode" only. I'm just curious if it is okay for me to use the product in this manner, as my goals are increased blood and lymphatic flow post exercise while I'm stuck sitting on a bus for a couple hours.

    Even though you might be getting a contraction, the TENS stimulates the nerves, where the EMS actually sends an impulse to contract the muscle directly. So I wouldn't recommend using the TENS to get a muscle contraction. However, a TENS will increase blood flow without the contraction if that's what you are looking to do. Good luck!

    You're welcome. The TENS doesn't make contractions, you might get twitches that feel like contractions, but it is not the same. If you feel like it's helping, then you can try it, but I don't think you are going to get the result you are looking for with the TENS, it will really only fatigue the muscle versus getting a true contraction for strengthening and re-education.
