Here's my interpretation of this song:
I once read one interpretation that explained this song as referred to his alcoholism, as in "make mine a pain in the neck"", "drink up so we can both finally die" or "why did she finally leave? 'cause all you think of is me." as if he was talking to the alcohol and it was replying... but in my opinion, it goes more to his depression.

I'm not even aware if he suffers from depression or not, but this to me is a song that acts as a chat with all his anxieties, fears and depressive thoughts. "make mine a pain in the neck", "drink up so we can BOTH finally die" and "why did she finally leave? 'cause all you think of is me", as to love someone else you must first love yourself, and depression in a couple makes one close himself up in the fear of not being enough or losing the other loved one, making the fear come true.
