The man on the cover has never been identified.

They just saw him at some Fat Festival in Virginia, and decided to take a picture of him back in 1983, years later they find the photo and put him on the cover because it matched the album perfectly.

Vague rumors have it that he died a while ago, but I don't believe anything without concrete evidence.

There were also rumors that Quentin Cook himself had a fat phase in the 80's and this was him before he lost a lot of weight, but he confirmed it's not true.
