@Erwin Woodedge It's a small joke: he is euphemistically saying
"Now I know that you're fucking whores..."
The rest of the song makes sense when you know that. Nothing to do with heroin.

I saw an EC solo show around the time this video was uploaded. When he started off with this song, I knew we were in for a fantastic show.
Awesome. Elvis at the peak of his lyrical and melodic powers. This album is impressive for the number and exquisite quality of all the songs but this is the icing on the cake.

But I don't get it. The other songs have a strong 60s R&B sound, as suggested, but this one still has that early new wave sound he tried to avoid.
